Family Card (KK): new households, changes and replacements
Prepare a Family Card application for changed details, a new household, or a lost or damaged KK, and check the local Dukcapil document requirements.
English edition. Official forms and source documents may be in Indonesian.
A Kartu Keluarga, usually called KK, records household members and their details. Choose the Dukcapil service for your actual reason: a new household, changed information, or replacement of a lost or damaged document. Supporting evidence depends on the change.

Guide contents
What to prepare
- The existing KK if available.
- The form matching the application reason.
- Evidence of the event or change, such as a marriage, birth or other relevant document.
- For loss, the police loss statement; for damage, the damaged KK, following local requirements.
Steps you can follow
Identify the reason
Describe exactly what needs to be created or changed.
Check local requirements
Read your district/city checklist and the relevant data-change form.
Prepare evidence
Use documents that establish the event or correct information.
Apply officially
Submit through the authorised channel and keep the reference number.
Check the result
Review spelling, relationships and every requested change on the new KK.
Costs and timing
Population-document services are free. Completion depends on local arrangements and whether the documents are complete.
Check current requirements, availability and charges through the official reference ↗ before paying or travelling.
The supporting document follows the change
Adding a child, recording a marriage, separating households and correcting a birth date are different situations. A single generic checklist may miss evidence needed for your case. Explain the change before submitting and retain originals as required.
For loss and damage, distinguish the evidence: a loss report is associated with a lost document, while a damaged document should be provided for a damage replacement. Use your local checklist for the full procedure.
Check linked services after the KK is issued
Review the document before using it elsewhere. Request any further correction through Dukcapil; do not manually edit a downloaded KK.
Make a list of services using the changed information, such as schools and BPJS Kesehatan. Check with each provider whether updates are required. A changed KK does not guarantee all other accounts update that day.
Things to check carefully
- Do not submit an unrelated form just because it is easier to find.
- Check names and family relationships before using the new document.
- Protect the personal details of all household members.
Frequently asked questions
Are requirements the same for every change?
No. The reason for applying determines the supporting documents.
What should I prepare for a lost KK?
Check the local replacement checklist, including the police loss statement. A damaged KK follows a different evidence route.
Will BPJS and school data update automatically?
Do not assume so. Confirm the update process with each service after checking the new KK.
